This is from the episode when Ryoga gets incredibly strong at the cost of looking like an idiot. Basically, he has a smiley face painted on his stomach that will only come off when he loses a fight.
At this point, Akane has seen both Ryoga and P-chan's stomachs, and being the brillant and deductive girl that she is in regards to her little pet, she has started wondering why the marks are exactly the same. In this cel, she is questioning Ranma, but as you can see from the look on his face, he's not about to tell her the truth.
