This is from the episode when Ryoga gets incredibly strong at the cost of looking like an idiot. Basically, he has a smiley face painted on his stomach that will only come off when he loses a fight.
Akane has noticed that Ryoga and P-chan have the exact same mark on their stomachs, and is starting to wonder why. In this cel, Ranma has refused to answer her questions, mainly because he is in fact responsible for Ryoga's pig curse, and thus honors Ryoga's request that Akane can never discover the curse. As you can see, she isn't too happy about Ranma's uncooperative nature.
